IAA Self Referral Information

For advice from the Early Years & Childcare Service, please read the guidance notes and then complete the referral form.

To make a referral, please download the following form and save it to your computer before filling it in.

Please remember you will need to submit the information by secure email to the correct inbox childcare.planning@suffolk.gov.uk.
Parental Consent Form - This form can be used for gaining parental permission  
Specialist Provision

The Inclusion Service Referral Form should be completed if there is evidence that indicates that a child may benefit from specialist provision. This may include specialist support to meet complex health needs or specific learning differences or a placement in a specialist nursery, specialist unit or special school

Education, Health & Care (EHC) Assessments
An Education, Health and Care (EHC) needs assessment can be requested by parents, carers, teachers or any other professional involved with a child or young person; they can also be requested by a young person over the age of 16.
Return the EHC form to the appropriate team: The form and any additional supporting reports and evidence should be emailed to your locality SEND Family Services Team: ** Remember to request a secure email first**
